Position Summary

Blue Ridge Care is seeking an experienced and strategic Director of Clinical Services to provide clinical, operational, and leadership oversight for Blue Ridge Independence at Home (BRIH) PACE. Reporting to the Executive Director of PACE, this role serves as the clinical leader and administrator for nursing and related clinical services, ensuring the delivery of high-quality, participant-centered care across the continuum of PACE services.

The Director of Clinical Services oversees clinical operations delivered in the PACE center, participants' homes, and other care settings while partnering closely with the Medical Director, interdisciplinary leadership team, and community partners to ensure exceptional outcomes, regulatory compliance, and operational excellence. While primarily focused on leadership and administration, this position may provide limited direct clinical support during urgent staffing or participant care needs.

Essential Duties and Responsibilities
  • Provide leadership and oversight for nursing, rehabilitation, social work, home care, personal care, medication management, and related clinical services.

  • Ensure consistent, coordinated, participant-centered care across all PACE service settings, including after-hours services.

  • Oversee clinic operations, home care services, triage processes, medication administration systems, clinical documentation, and participant care workflows.

  • Collaborate with the Medical Director and primary care providers to establish clinical guidelines, standards, and best practices.

  • Provide leadership, supervision, coaching, and performance management to assigned clinical and interdisciplinary team members.

  • Support recruitment, onboarding, orientation, competency validation, and ongoing staff development.

  • Assess staffing levels and operational needs to support participant care, program growth, and organizational goals.

  • Serve as an active member of the PACE leadership team and contribute to strategic planning, program development, and organizational initiatives.

  • Develop and implement departmental goals, work plans, performance measures, and process improvement initiatives.

  • Lead quality improvement efforts, audits, corrective action plans, incident reviews, and performance improvement projects.

  • Ensure compliance with all CMS, DMAS, PACE, state, federal, and organizational regulations and standards.

  • Develop, maintain, and monitor clinical policies, procedures, and operational workflows.

  • Oversee contracted clinical services, vendors, and community partners to ensure quality, efficiency, and compliance.

  • Monitor departmental budgets, staffing resources, supplies, equipment, and expenditures.

  • Promote participant dignity, safety, independence, privacy, and quality of life.

  • Participate in clinical on-call responsibilities and provide support for urgent participant care concerns as needed.

  • Build collaborative relationships with participants, families, caregivers, staff, providers, and community partners.

  • Maintain accurate documentation and ensure audit readiness of participant records and clinical operations.

  • Support emergency preparedness, infection prevention, workplace safety, and risk management activities.

  • Perform other duties as assigned.

Education and Experience
  • Bachelor of Science in Nursing (BSN) from an accredited nursing program required.

  • Current, unrestricted Registered Nurse (RN) license in the Commonwealth of Virginia required.

  • Current CPR certification required.

  • Minimum of three years of recent clinical nursing experience required.

  • Minimum of two years of nursing leadership or management experience in a hospital, nursing facility, PACE, home care, community health, or similar healthcare setting required.

  • At least one year of experience working with frail or older adult populations required.

  • PACE, geriatric care, long-term care, home health, or community-based healthcare experience strongly preferred.

  • Valid driver's license, proof of insurance, and reliable transportation required.

  • Must meet immunization, health screening, communicable disease, and regulatory requirements.

Skills and Knowledge
  • Strong knowledge of clinical operations, nursing practice standards, quality improvement, and healthcare regulations.

  • Comprehensive understanding of interdisciplinary care and participant-centered service delivery.

  • Demonstrated leadership, coaching, staff development, delegation, and performance management skills.

  • Strong clinical assessment, triage, problem-solving, and decision-making abilities.

  • Ability to interpret and implement regulations, policies, procedures, and professional standards.

  • Excellent communication, relationship-building, and presentation skills.

  • Ability to manage multiple priorities and perform effectively in a fast-paced, evolving healthcare environment.

  • Strong organizational skills with experience managing budgets, staffing resources, and operational objectives.

  • Proficiency with electronic health records and Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook.

  • Commitment to collaboration, accountability, confidentiality, and continuous improvement.

Work Environment & Physical Requirements
  • Works in healthcare, office, participant home, and community-based settings.

  • May be exposed to bodily fluids, communicable diseases, biohazardous materials, and other healthcare-related risks.

  • Requires local travel throughout the service area and occasional evening, weekend, and after-hours responsibilities.

  • Ability to stand, walk, bend, lift, carry, push, and pull as required to perform duties safely.

  • Must be able to lift or carry up to 50 pounds and push or pull equipment exceeding 150 pounds with appropriate assistance and equipment.

  • Requires vision, hearing, manual dexterity, and communication abilities sufficient to perform assigned responsibilities.

  • Must be able to work effectively under stressful conditions and respond appropriately to urgent participant care situations.
