About the Role

In this role, you will conduct comprehensive health assessments during visits that may last up to one hour. You will evaluate members’ current health, medical history, medication adherence, and social environments. Your work provides critical insights that enhance the quality of care, allowing us to address multiple care needs efficiently.

• Perform thorough assessments for adults and geriatric members, including detailed health histories, diagnoses, treatments, and health education.
• Order necessary lab-based tests and conduct point-of-care testing.
• Collaborate effectively with physicians, case managers, social workers, and family members to ensure holistic patient care.
• Deliver services at the member's place of residence, whether at home or in Skilled Nursing Facilities.

• Competitive Pay: Your income is based on the number of assessments completed, with potential earnings ranging from $1,800 - $11,500 per month based on a rate of $90 - $115 per assessment.

Qualifications

• Master's Degree or equivalent experience with NP licensure.
• Current NP licensure in applicable state, including prescriptive authority or eligibility for such.
• Board certification from AANP, AACN, or ANCC in relevant specialties.
• Valid BLS, ACLS, or CPR certification.
• Preferred: 1 year of NP experience; new graduates are welcome to apply!
• Valid state driver's license and automobile insurance coverage.
• Computer proficiency and familiarity with health/medical record software.
• Strong verbal and written communication abilities.
