Palliative Care Nurse

Join Our Compassionate Team as a Palliative Care Nurse Practitioner!

We are seeking a skilled Palliative Care Nurse Practitioner to provide advanced care to patients in their homes, ensuring they receive the highest quality of life while effectively managing complex medical needs. You will work closely with the Palliative Medical Director and operate independently to support patients and their families through every phase of treatment.

Your Responsibilities Will Include:

• Collaborating closely with the Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care.

• Conducting thorough assessments, including physical, emotional, and psychosocial evaluations.

• Creating and implementing customized care plans in partnership with patients and their families.

• Managing treatment plans and adjusting them as needed based on patient progress and needs.

• Prescribing medications (including controlled substances) and ordering diagnostic tests as permitted by state law.

• Maintaining precise clinical documentation according to professional standards.

• Communicating effectively with physicians, caregivers, and other healthcare professionals.

• Participating in evening/weekend on-call rotation as necessary.

• Facilitating referrals for additional care services when appropriate.

About You:

We are looking for someone with:

• Strong leadership skills and excellent interpersonal communication abilities.

• Proven ability to make independent decisions and demonstrate self-motivation.

• A solid understanding of palliative medicine principles, home health, hospice eligibility, and Medicare/Medicaid regulations.

• Familiarity with the interdisciplinary care process.

• Commitment to continuous education requirements for Gentiva and licensing boards.

• Proficiency in point-of-care software and technology.

Qualifications:

• A Master of Science in Nursing from an accredited institution (or equivalent as per state law).

• A minimum of two years' experience as a Nurse Practitioner.

• Specialization in Family or Adult Medicine is preferred.

• Experience with Part B billing.

• The ability to work autonomously in an advanced practice role.

Licenses and Certifications:

• Current RN license and Nurse Practitioner certification as required by state law.

• Willingness to establish a collaborative agreement with a supervising physician.

• Part B Provider certification.

• Federal DEA license and state DEA license (if applicable).

• ACHPN certification in Hospice and Palliative Care is preferred.

• A valid driver's license and current automobile insurance.
