Job Title: Nurse Practitioner Outpatient Clinic

Location: Lynchburg, VA

Schedule: Monday Friday, 8:00 AM 4:30 PM

Patient Load: 10 12 patients per day

Start Date: 1 2 months

We are seeking a dedicated Nurse Practitioner (NP) to join our outpatient clinic in Lynchburg, VA. This role focuses on providing high-quality care in a supportive environment, with a manageable patient volume that allows for thorough and personalized treatment.

Key Responsibilities:

• Provide comprehensive care to patients with chronic disease conditions.
• Conduct physical exams, patient assessments, and ongoing management of treatment plans.
• Collaborate with physicians and the care team to deliver excellent patient outcomes.
• Maintain accurate and timely documentation within the clinic's EMR system.

Qualifications:

• Licensed Nurse Practitioner in Virginia (or ability to obtain).
• Strong interest or experience in chronic disease management .
• Excellent communication and patient-centered care skills.
• New graduates are encouraged to apply mentorship and support provided.
