Care Advantage, Inc. is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our growing High Acuity Division. In this specialized role, you'll provide individualized, one‑on‑one care for medically fragile adults who require focused attention, clinical oversight, and emotional support in the comfort of their own homes.
Our High Acuity program is designed to reduce depression, slow cognitive decline, and help patients safely maintain independence with customized care plans developed by our Registered Nurses. If you're a skilled LPN who is passionate about meaningful patient relationships and delivering exceptional care, this is an opportunity to make a true difference.

What You'll Do (Expanded LPN Responsibilities)
As a High Acuity LPN, you will play a central role in delivering comprehensive, personalized care. Your work will support both the patient's physical stability and their emotional well‑being.
Direct Patient Care
- Provide hands‑on nursing care including vital signs monitoring, symptom observation, and basic clinical procedures
- Follow and implement RN‑developed care plans tailored to each patient's unique needs
- Assist with mobility, medication reminders, and condition‑specific care tasks
- Promote patient comfort, safety, and emotional stability through attentive, individualized care
Accurate Clinical Documentation
- Complete timely, clear documentation that reflects progress, changes in condition, and care interventions
- Communicate concerns or updates to the RN Case Manager or Clinical Supervisor
Family & Caregiver Education
- Teach families about daily care routines, safety practices, and important warning signs
- Help caregivers build confidence and competence in supporting their loved one
Specialized Support & Procedures
- Assist RNs or physicians with specialized tasks, including:
- Preparing treatment equipment
- Maintaining aseptic technique
- Monitoring responses to therapies
- Support the patient in learning safe self‑care techniques when appropriate
Team Collaboration
- Participate in coordinated home health services
- Identify when additional disciplines may be needed (HHA, OT, PT, MSW, ST, Dietician) and report accordingly
- Foster strong communication with supervisors to ensure a seamless care experience
What You Need to Qualify
We are looking for someone who is both clinically capable and emotionally grounded:
- Eligibility to work in the United States
- Active LPN license (minimum 1 year of LPN experience)
- CPR Certification and up‑to‑date PPD
- Valid driver's license
- Reliable transportation
- Strong communication skills and a commitment to compassionate, patient‑first care
