Hours : Full-time – 37.5 hours per week (Monday through Friday, 8:30 a.m. – 5:00 p.m.)
Location : Lynchburg VA, 24501
Compensation dependent on experience
Position Summary
The Case Manager promotes and supports individuals who are at risk of developing or who have mental health and/or substance use issues. Services include assessing, monitoring, and linking to services, supportive level counseling, education, and advocacy. Case management services may also be provided to a small number of priority population women with substance use disorders as needed.
Required Education and Experience
- Bachelor’s degree from an accredited college or university in an applicable human services field; or relevant education which indicates that the individual possesses the required knowledge, skills, and abilities.
- A minimum of one (1) year of relevant mental health, developmental disability, and/or substance use work experience with the identified population, or a combination thereof that demonstrates that the candidate possesses the required knowledge, skills, and abilities; and/or an equivalent combination of education, training, and experience that provides the individual possesses the required knowledge, skills, and abilities.
Related Knowledge, Skills and Abilities
- Services, systems, and programs available in the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, developmental disabilities, and substance abuse treatment programs.
- The nature of serious mental illness, developmental disabilities, and substance abuse depending on the population served, including clinical and developmental issues.
- Different types of assessments, including functional assessments, and their uses in service planning.
- Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.
- The service planning process and major components of a service plan.
- The use of medications in the care or treatment of the population served.
- All applicable federal and state laws, state regulations, and local ordinances.
- Identifying and documenting an individual's needs for resources, services and other supports.
- Using information from assessments, evaluations, observation, and interviews to develop individual service plans.
- Identifying services and resources within the community and established service system to meet the individual's needs; and documenting how resources, services, and natural supports, such as family, can be utilized to achieve an individual's personal habilitative/rehabilitative and life goals.
- Coordinating the provision of services by public and private providers.
- Work as team members, maintaining effective inter- and intra-agency relationships.
- Work independently, performing position duties under general supervision.
- Engage and sustain ongoing relationships with individuals receiving services.
- To drive and must possess a valid Virginia driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to clients at other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites.
- To participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
- Willing and available to work modified schedules (weekends, holidays, etc.) as required.
- Demonstrate a positive regard for individuals and their families (e.g., allowing risk taking, avoiding stereotypes of developmentally disabled people, respecting individuals' and families' privacy, believing individuals can grow).
