Location: Field-Based (Patient Homes throughout Virginia)

Position Summary

Vision Healthcare, LLC is seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to provide high-quality skilled nursing services in patients' homes. The Registered Nurse is responsible for performing comprehensive patient assessments, developing and implementing individualized plans of care, providing skilled nursing treatments, educating patients and caregivers, coordinating care with physicians and the interdisciplinary team, and ensuring compliance with all applicable federal, state, and agency regulations.

The RN plays a critical role in promoting patient independence, preventing hospitalizations, improving health outcomes, and delivering exceptional care consistent with Vision Healthcare's mission of providing compassionate, patient-centered healthcare in the home.

Essential Duties and Responsibilities

The Registered Nurse is responsible for, but not limited to:

Patient Care

  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification (RECERT), Routine Skilled Nursing, Supervisory, Transfer, and Discharge visits.
  • Complete comprehensive nursing assessments and ongoing patient evaluations.
  • Develop, implement, and revise individualized Plans of Care (POC) in collaboration with the physician.
  • Monitor patient progress and communicate significant changes in condition promptly.
  • Provide skilled nursing services in accordance with physician orders and agency policies.

Skilled Nursing Services

  • Medication management and patient education.
  • Intravenous (IV) therapy and home infusion services.
  • PICC line, Midline, Central Venous Catheter, and Implanted Port care.
  • Wound assessment and wound care, including Negative Pressure Wound Therapy (Wound VAC).
  • Catheter insertion, maintenance, and Foley catheter care.
  • Ostomy care and education.
  • Tracheostomy care and suctioning (when applicable).
  • Enteral feeding tube management.
  • Blood specimen collection and laboratory coordination.
  • Diabetic education and insulin administration.
  • Disease management for chronic and acute medical conditions.
  • Infection prevention and control.

Care Coordination

  • Collaborate with physicians, pharmacies, therapists, case managers, hospitals, caregivers, and other healthcare professionals.
  • Coordinate patient services to ensure continuity of care.
  • Participate in interdisciplinary team meetings and case conferences.
  • Obtain and communicate physician orders as necessary.

Documentation & Compliance

  • Complete all clinical documentation accurately and within agency-required timeframes.
  • Maintain complete and timely documentation in the agency's Electronic Medical Record (EMR).
  • Ensure documentation supports medical necessity and payer requirements.
  • Maintain compliance with:
  • Virginia Department of Health (VDH)
  • Virginia DMAS regulations
  • Medicare and Medicaid requirements
  • HIPAA Privacy and Security Rules
  • Agency policies and procedures

Patient & Caregiver Education

  • Educate patients and caregivers regarding:
  • Disease processes
  • Medication management
  • Fall prevention
  • Infection prevention
  • Home safety
  • Equipment usage
  • Nutrition and hydration
  • Emergency preparedness
  • Self-management of chronic conditions

Quality & Professional Responsibilities

  • Maintain patient confidentiality.
  • Participate in orientation, competency validation, annual education, and required in-service training.
  • Maintain professional communication with patients, families, referral sources, and coworkers.
  • Supervise Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Personal Care Aides (PCAs), and Home Health Aides (HHAs) as assigned.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Perform other duties as assigned by the Director of Nursing.

Required Qualifications

  • Graduate of an accredited School of Nursing.
  • Current unrestricted Registered Nurse (RN) license in the Commonwealth of Virginia.
  • Current CPR/BLS certification.
  • Minimum one (1) year of nursing experience required.
  • Home Health experience preferred.
  • Home Infusion experience preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent verbal and written communication skills.
  • Ability to work independently with minimal supervision.
  • Proficient computer and electronic documentation skills.

Licenses & Certifications

Must maintain current:

  • Virginia Registered Nurse License
  • CPR/BLS Certification
  • Valid Driver's License
  • Current Automobile Insurance
  • Reliable Transportation

Professional Liability Insurance may be required based on employment classification and agency policy.

Physical Requirements

The employee must be able to:

  • Frequently stand, walk, bend, kneel, squat, and reach.
  • Lift, push, or move up to 50 pounds with or without reasonable accommodation.
  • Travel throughout the assigned service area.
  • Safely transfer and assist patients when necessary.
  • Utilize clinical equipment and electronic documentation devices.

Work Environment

This position is performed primarily in patients' homes throughout the agency's service area. Travel between patient homes is required. The RN may occasionally attend meetings, training sessions, or complete documentation at the agency office.

Requirements:

Required Qualifications

Graduate of an accredited School of Nursing.
