Maternal and Child Health (MCH) Referrals Coordinator 25-051
Summary
The Maternal and Child Health (MCH) Referrals Coordinator will play a pivotal role in improving maternal and infant health outcomes in the Eastern Region of Virginia, in alignment with Virginia Title V objectives. Reporting to the Associate Director of the Minus 9 to 5 as an employee of the Old Dominion University Research Foundation, the MCH Referrals Coordinator will be responsible for establishing and maintaining a centralized referral mechanism for MCH home visiting services, tracking and managing data related to client services and maternal and infant outcomes, and enhancing community engagement to support MCH initiatives. This position is designed to reduce maternal morbidity and mortality, increase community engagement, and foster regional collaboration, in accordance with the goals of Virginia Title V.
Job Responsibilities:
Establish and Maintain a Centralized Referral Mechanism for MCH Home Visiting Services:
- Develop, implement, and manage a centralized, user-friendly referral system that connects pregnant individuals and families to appropriate MCH home visiting services across the Eastern region, improving access and timely intervention.
- Ensure the referral process is equitable and easily navigable, addressing barriers to access for underserved and vulnerable populations.
- Foster collaboration among community-based organizations, healthcare providers, and local agencies to ensure smooth referrals and coordinated care.
- Continuously assess and refine the referral system to enhance service delivery, reduce delays, and align with Virginia Title V’s goals of reducing maternal morbidity and mortality.
Track Data Across Coalition Partners:
- Collect, track, and report from Minus 9 to 5 coalition partners and all regional Title V recipients, on selected Title V metrics, including the number of clients served, postpartum visits completed, and key maternal and infant health outcomes.
- Support preparation of regional data reports and performance evaluations that provide insight into the effectiveness of MCH services, contributing to the Title V objectives of improving maternal health outcomes and reducing mortality.
- Use data from referrals to assess and identify regional service gaps, disparities, and opportunities for targeted improvements in home visiting service provision, particularly in communities most at risk.
- Ensure data quality and integrity, adhering to best practices for health data management and reporting, in alignment with Title V’s requirements for measurable impact and accountability.
Increase Community Engagement in MCH Initiatives:
- Actively engage and educate local community members, healthcare providers, and stakeholders about MCH services and the Title V objectives to improve maternal and child health outcomes.
- Promote and increase awareness of MCH services and resources available through Minus 9 to 5, particularly focusing on the goal to reduce maternal morbidity and mortality.
- Foster community-driven solutions to address barriers to care and encourage participation in preventive health measures to improve maternal and infant outcomes.
Enhance Regional Collaboration to Achieve Title V Objectives:
- Support the efforts of Minus 9 to 5 to facilitate collaboration across health systems, organizations, and regional partners to create a more integrated approach to MCH service delivery, ensuring that regional efforts are aligned with Title V objectives.
- Serve as a liaison between Minus 9 to 5and VDH Local Health District Title V grant recipients, with an emphasis on improving access to care for maternal and child health populations.
- Support Minus 9 to 5 in advocating for the adoption of best practices and innovative solutions to address regional disparities in maternal and infant health, aligning efforts with Virginia’s Title V priorities
Education and/or training:
- Master’s degree in Public Health, Social Work, Nursing, or a related field.
- Strong understanding of maternal and child health issues, including the social determinants of health and health disparities, as well as Title V program goals.
- Experience with data collection, analysis, and reporting in public health or health services settings.
- Ability to build and maintain collaborative relationships with diverse stakeholders, including healthcare providers, community-based organizations, and government agencies.
- Excellent communication, organizational, and problem-solving skills.
- Proficiency with Microsoft Office Suite, data management software, and other relevant technology tools.
Level and type ofexperience
- At least two years of experience in maternal and child health, public health, or a related field
