Supervisory Social Worker (Patient Aligned Care Team)

Summary

The incumbent works for the Fredericksburg Health Care System (FxHCC) in Fredericksburg - Virginia - as a direct report to the Patient Aligned Care Team (PACT) and Associated Programs Social Work Supervisor.

Duties

• This position provides supervision to the PACT social work team and will further manage the program needs of the PACT social workers - to include collaborating with the interdisciplinary leadership for PACT at the Fredericksburg HCC
• The incumbent will ensure that PACT social work services and documentation meet National and Local Social Work Guidelines - Adhere to Medical Center Policies (MCP) and Standard Operating Procedures (SOP)
• The incumbent will provide direct clinical social work services in the PACT department including biopsychosocial assessments - case management and crisis intervention
• Performs other directly related duties as assigned
