Responsibilities

About the Role

As a Matrix provider, you will conduct in-depth health assessments that last up to one hour, allowing for a thorough review of a member's health status and needs. Your role includes:

• Performing comprehensive adult/geriatric health assessments including medical history, diagnosis and treatment plans, health education, and appropriate referrals.
• Administering and ordering necessary labs and point-of-care testing.
• Collaborating effectively with a team of physicians, case managers, social workers, family members, and caregivers to enhance patient care.
• Providing services directly at the member's residence, whether it be their home or a Skilled Nursing Facility.

Qualifications

Skills & Experience That Stand Out

• Master's Degree or equivalent experience with successful NP licensure.
• Current NP licensure with prescription authority or ability to obtain it.
• AANP, AACN, or ANCC board certification in relevant specialties.
• Current BLS, ACLS, or CPR certification.
• Preferred 1 year of NP experience; new graduates are encouraged to apply!
• Valid state driver's license and automobile insurance that meets state requirements.
• Proficient computer skills and experience with medical record software.
• Excellent verbal and written communication abilities.
