Required Education and Experience:

• Must be licensed in the State of Virginia as a Practical Nurse (LPN) is required.
• Experience in psychiatric nursing care preferred.
• Responsible for case management and nursing care services under the instructions of a licensed physician and registered nurse to individuals with mental illness(es), intellectual disabilities, and/or substance abuse.

Position Description:

The Licensed Practical Nurse (LPN) is responsible for case management and nursing care services under the instructions of a licensed physician and registered nurse to individuals with mental illness(es), intellectual disabilities, and/or substance abuse. This includes clinical assessment of case management and nursing needs, treatment planning, linking with community services, aggressive outreach and follow-up and monitoring of clients. This position will combine the aspect of medication monitoring and compliance with active case management services. He/she is supervised by a qualified mental health professional in conjunction with the agency psychiatrist and registered nurse and/or Director of Long Term Care Services or designee. Staff shall be hired with the goal of becoming welcoming, recovery-oriented, and co-occurring competent to engage all individuals receiving services in empathetic, hopeful, integrated relationships to help them address multiple issues successfully and shall support agency efforts to accomplish this.

Related Knowledge, Skills, and Abilities:

Services and systems available in the community including primary health care, support services, eligibility criteria and intake processes and generic community resources. Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning and service coordination. Responsible for the administering of medications to clients as per psychiatrist order and per agency policy and procedures. Responsible for assessment and identification of client service treatment needs. Responsible for supportive counseling to individuals, families and groups to maintain client stabilization and functioning ability and referral to appropriate services provider(s) based on client's need for services. Responsible for development of treatment planning and coordination with other service providers.
