Key responsibilities:
- Implement and monitor individualized physical therapy plans under the supervision of licensed clinicians, applying evidence-based techniques to restore function, decrease pain, and improve mobility.
- Conduct patient evaluations, document progress, and adjust treatment plans as goals evolve, ensuring clear communication with patients, families, and the care team.
- Facilitate therapeutic activities and modalities, teach home exercise programs, and provide patient and caregiver education to promote adherence and safety.
- Assist in discharge planning, coordinate with multidisciplinary teams, and participate in quality improvement initiatives that elevate patient outcomes.
