CASE MANAGER III - NEW HORIZONS

New River Valley

New River Valley Community Services is seeking a Case Manager III at New Horizons! The Case Manager III provides case management for clients of the crisis stabilization program, including clients with mental illnesses and co-occurring substance abuse. The Case Manager coordinates services, resources needed by the client during residential services, and monitors outcomes up to 30 days post discharge. This role also provides coordination for discharge services with agency and community resources. In this position, you will have the opportunity to make a positive impact on the lives of those experiencing a mental health crisis within our community!

Starting Salary: $37,072-Depending on Experience

Education Requirements:

Bachelor's Degree in Social Work, Psychology or other related field is required.
Registration as a QMHP-A is required. Must maintain QMHP status and all required trainings/CEUs.
