Responsibilities
- Respond to hospice-related calls by phone or arrange appropriate care, including home visits or visits to care sites as needed.
- Provide education and emotional support to patients and families navigating end-of-life care.
- Document all activity electronically and complete on-call shift notes prior to reporting off.
- Collaborate with the interdisciplinary team to coordinate symptom management, plan of care, and care transitions.
- Adhere to hospice guidelines, safety protocols, and professional standards in all patient interactions.

Requirements
- Active Virginia Registered Nurse (RN) license
- Minimum 1 year hospice experience
- Valid driver’s license with a good driving record and reliable transportation
- Ability to work in a home healthcare - hospice setting
- Accepts on-site travel for patient visits as part of a travel assignment
