In this role you’ll own key responsibilities that are central to excellent LTC outcomes and ongoing professional growth. Conduct comprehensive assessments, interpret evolving clinical data, and contribute to clear, collaborative care plans with physicians and the broader care team. Administer medications with precision, manage wounds and infections, and support mobility, daily living activities, and fall prevention to promote safety and independence. You’ll coordinate with therapy, social work, dietary services, and families to ensure holistic, resident-centered care that aligns with stated goals and preferences. As a clinical mentor, you’ll guide nursing assistants and new staff, share best practices, and participate in quality improvement initiatives that elevate resident satisfaction and safety metrics.
